تعریف خطای انسانی
Definition of Human Error
یك تصميم نامناسب و يا نامطلوب و يا رفتاری كه مي‌تواند باعث كاهش اثربخشی، ايمنی و يا عملكرد سيستم شود.
An inappropriate or undesirable human decision or behavior that reduces or has the potential to reduce effectiveness, safety, or system performance.
فعاليت و يا تصميمي كه بيش از توان تحمل سيستم است
A human action/decision that exceeds system Tolerances.
عملي كه يكي از شرايط زير را دارا باشد:
- با بي توجهي صورت گرفته باشد
- مغاير استانداردها يا قوانين باشد
- خارج از حد قابل قبول سيستم مي باشد
“An action is taken that was:
- not intended by the actor;
- not desired by a set of rules or an external observer;
- or that led the task or system outside its acceptable limits”

خطای ارتکاب: افزودن یا وارد کردن چیزی که نباید آنجا باشد.
غفلت یا سهل‌انگاری: از نظر انداختن چیزی مثل یک مرحله از یک سلسله مراحل
انتخاب: انتخاب نادرست از میان مجموعه‌ای از گزینه‌ها
توالی: قرار دادن نادرست یک سلسله از فعالیت‌ها یا رخدادها
زمان: انجام بسیار دیر یا بسیار زود یک فعالیت
کیفی: انجام فعالیت به شکل نامناسب

- نفوذ- ورود به منطقه خطرناك
• Intrusion – entering a dangerous area / location

- برگشت به عقب- تلاش براي يك فعاليت آغاز شده
• Reversal – trying to stop or undo a task already initiated

• Disordering – task or set of task performed in the wrong sequence
- از دست دادن زمان- عدم توانايي در انجام كار با زمان تعيين شده
• Mistiming – person fails to perform the action within the time allotted

خطاهاي مهارتي
Skill-based errors
- اين خطاها در طول اجرا مشخص شده و به وجود مي آيند
• These arise during the execution of a well-learned, fairly routine task.
- خطاهاي مبتني بر مهارت معمولا در بين افراد ماهر سازمان مشاهده مي شود
• Skill-based errors commonly occur among st the more highly skilled members of the organization.
- بسياري از افراد ماهر به دليل توانايي كه دارند فك می‌كنند رعايت الزامات و اقدامات ايمني مختص افراد كم تجربه است
• Such people, because of their acquired skills and experience in the work process, frequently take the view that the safety precautions are purely for the benefit of the under skilled and under experienced operator.
- این نگرش باعث بروز حوادث به ويژه هنگام كار با ماشين الات براي افراد ماهر مي شود.
• Such an attitude to work is accompanied by overconfidence in doing the task which can lead to, particularly, machinery-related accidents.
خطاهاي مرتبط با نقش
Rule-based errors
- اين خطا معمولا وقتي اتفاق مي افتد كه مجموعه اي از دستورالعمل ها و يا قوانين مشابه براي هدايت و انجام فعاليت ها استفاده مي شود.
• These are the types of error which occur when a set of operating instructions or similar set of rules is used to guide the sequence of actions.
- خطاهاي مرتبط با نقش وابستگي زيادي به زمان،ويژگي هاي فردي ،جسمي ،ذهني و احساسي افراد دارد.
• They are more dependent on time, on individual cast of mind and on the temporary physical, mental or emotional state of the person concerned.
- اين خطاها تقريبا غيرقابل پيش بيني و آزمايش هستند.
• They are, therefore, harder to predict from past statistics or experiment.
